have you setup the dimension to 50 on avatars and saved it, or you have only the default placeholder 50, but you still have not set it and save? This the problem i think about dimension.
About galleries in phpBB, we can setup more than one? If yes so i need to check how the joke is in phpBB about this, and all will be perfect.
You have more than one gallery image for avatars in phpBB, isn't it?
So i'll setup another gallery avatar in phpBB if it is possible, so i'll test out the joke: after all will be perfect, as is all should work OK by the way. As soon i can i'll setup this gallery more (is it possible without a mod in phpBB??) to see how to fix on fly.

Well, nothing, i've just setup two different galleries into phpBB, and the code fix i've add last night to patch 1.5.0 at https://wordpress.org/plugins/wp-w3all- ... tegration/ repository, work ok.
This about a default phpBB3 install. Work ok on my test.
Are you using some gallery mods in phpBB?
i'll setup here on forum, a gallery or two for avatars, so would be possible to test it out here also, looking for any bug just on place, if there is still something wrong.

have you setup BLANK avatars as default for users that have not one in WP admin -> Settings -> Discussion ???

Because the output of the img for users in WP, that do not have a Gravatar, say that. It is correct, and not a css problem.
A test gallery has been setup here as example, enabled on forum to test online.
All features on phpBB enabled about avatars, all return and work fine.
p.s look that in this setting you'll see all avatars the same, your if you have one, this is ok, it is not a problem, and YOU NEED to setup TO another option, not BLANK for guests or users that do not have an avatar on WP or phpBB.
