I tested it in any way, and effectively maybe the avatar do not update immediately, but after some browser's refresh, it display to me the new one not cached, both activating or not the following options:

Maybe i've been able to reproduce the issue, that's not about the cache, let see how to fix the problem!

I have this script "WP Fastest Cache", I have it deactivated and it still doesn't work well.

You don't mention if you are using or not a cache plugin, that could be the unique reason, if the scenario is that the user display a phpBB avatar in WP, but when updated in phpBB it do not update also in WP.

Yes, I have it activated.
I don't understand why it doesn't update only the image but pressing CTRL + F5 updates it.
I'm kind of giving up on this detail :(

Into the integration plugin admin page, both
[b][i]Use phpBB avatar to replace WordPress user's avatar[/i][/b]
and
[b][i]Activate phpBB avatars on widgets[/i][/b]
options require to be set to YES.
Maybe this is not into your configuration?
